Gastric Ulcer vs Duodenal Ulcer: How to Tell Them Apart

Both are peptic ulcers—mucosal defects extending through the muscularis mucosae—driven by the same core insults (H. pylori, NSAIDs) when attack overwhelms mucosal defense. The axis that separates them is the pain-food relationship and the underlying acid/H. pylori pattern: gastric ulcers hurt with food and carry cancer risk, while duodenal ulcers are relieved by food and are rarely malignant.

  • Endoscopy for alarm features (bleeding, weight loss, persistent vomiting, dysphagia, age >55 with new dyspepsia): mandatory for gastric ulcers to rule out cancer; used in duodenal ulcers mainly when patients fail empiric therapy or develop alarm symptoms.

  • Overlooking Zollinger-Ellison syndrome in refractory, multiple, or atypically located ulcers.
  • Misreading complication risk by location—posterior duodenal ulcers bleed (gastroduodenal artery) whereas anterior duodenal ulcers tend to perforate.
