Bacterial vs Viral Meningitis: How to Tell Them Apart
Both bacterial and viral meningitis present with headache, fever, and neck stiffness from meningeal inflammation, but they sit at opposite ends of the severity and urgency spectrum. The core axis that separates them is the CSF profile—neutrophilic pleocytosis with low glucose in bacterial disease versus lymphocytic pleocytosis with normal glucose in viral disease—paired with how sick the patient looks. Bacterial meningitis is a true emergency; viral (aseptic) meningitis is usually benign.

Remember that the full triad of fever, neck stiffness, and altered consciousness is present in only about 44% of bacterial cases, so a low threshold to perform LP is essential. Because bacterial meningitis is a medical emergency, give empiric antibiotics (plus dexamethasone) immediately for any suspected bacterial case—never delay treatment for CT or LP.

- Lumbar puncture with CSF analysis: bacterial shows neutrophils, low glucose, high WBC, and elevated protein; viral shows lymphocytes, normal glucose, and normal or mildly elevated protein.

- Delaying empiric antibiotics and dexamethasone for CT or LP in suspected bacterial meningitis; treatment must not be postponed.
- Overlooking altered mental status: it belongs to the bacterial triad, and marked altered consciousness should raise concern for encephalitis rather than simple viral meningitis.
